メタボリック症候群と非アルコール性脂肪性肝疾患

■近年，メタボリック症候群の肝病変の一つとして非アルコール性脂肪性肝疾患（NAFLD）に注目が集まっている．その一部の症例は炎症，線維化を伴う非アルコール性脂肪肝炎（NASH）であり，肝硬変から肝不全や肝癌まで進展する．

■NAFLDの病態形成の背景には肝臓，脂肪組織，筋組織を循環する脂質の供給・代謝のバランスの破綻があり，肝細胞内脂質沈着のメカニズムはインスリン抵抗性の誘導にも深く関与している．

■NASHの発症においては，肝臓内のイベントのみならずアディポサイトカイン（脂肪組織由来のサイトカイン）の発現変化が寄与していると考えられている．
