抗リン脂質抗体と血栓形成

●抗リン脂質抗体症候群(APS)は,リン脂質とリン脂質結合蛋白質の複合体に対する自己抗体である抗リン脂質抗体(aPL)が血中に検出され,動静脈血栓症や妊娠合併症を主症状とする症候群である.

●APSの臨床症状は,深部静脈血栓症や虚血性脳卒中が多く,急性心筋梗塞は少ない.

●APS分類基準は確定診断に用いられるが,診断のための検査所見が12週間以上はなれて2回以上検出されることが条件であるため,治療開始基準には不適である.

●APSの血栓機序は,aPLによる凝固制御機構の阻害などによる易血栓性の環境形成に加えて,aPL結合により活性化された血管内皮細胞上に組織因子などの凝固促進物質の発現を促すことによると推定される.
