●基質特異性拡張型βラクタマーゼ(ESBL)は,オキシイミノ基を有する第三世代・第四世代セファロスポリン系薬およびモノバクタム系薬を分解し,これらの抗菌薬に対する耐性を付与する.
●ESBLの主要産生菌は大腸菌,肺炎桿菌,Proteus mirabilisなどの腸内細菌科細菌である.
●ESBL産生菌は,院内・外来のいずれからも分離される耐性菌である.
●検出される主要なESBLはCTX-M-型であり,同酵素産生大腸菌の多くがフルオロキノロン系薬にも耐性を示す.
