Point

●慢性リンパ性白血病（CLL）の診断には，異常Bリンパ球数5,000/μL以上であることが必須の所見である．これを満たさないBリンパ球増多は単クローン性Bリンパ球増多症（MBL）と定義される．MBLにおいては白血球数の多寡が重要であり，白血球数の多いMBLはCLLに移行する可能性が高い．また，リンパ節病変のみのMBLも存在する．

●前がん病変と考えられる（in situ）濾胞性リンパ腫，マントル細胞リンパ腫について，それぞれ名称がISFN（in situ follicular neoplasia），ISMCN（in situ mantle cell neoplasm）と改められた．

●小児濾胞性リンパ腫は小児および若年成人に発症し，限局病変であることが多く，予後良好である．小児および若年成人に発症し，ワルダイエル輪，頸部に限局するリンパ腫で，IRF4（MUM1）遺伝子の再構成を伴う一群があり，新たな疾患概念として提唱されている．

●マントル細胞リンパ腫（MCL）は古典的MCLとそれ以外に大別され，後者は白血病化をきたしやすいが比較的緩徐な経過を示すことが多い一群とされる．
