椎体炎（2015年，IDSA）

POINT

●発熱と腰痛を訴える患者では，椎体炎（NVO）の存在を疑う．また，特にStaphylococcus aureus菌血症の既往が1年以内にある患者が腰痛を訴えた場合は，NVOの存在を疑う．

●NVOを疑ったときは脊椎をMRIで評価する．

●NVOの治療には微生物学的診断が不可欠であり，微生物学的診断がつくまでは経験的な抗菌薬の投与は控えるべきである．

●微生物学的な診断のためには血液培養2セットと，可能であればイメージガイド下の穿刺で生検を行う．

●NVOに対する抗菌薬の投与期間は6週間が標準となりつつあるが，重症例についてはより長期投与を推奨する意見もある．
