結核の診断（2016年，IDSA/CDC/ATS）

POINT

●今回の本ガイドラインで，インターフェロンγ遊離試験（IGRA）や分子生物学的手法を用いた診断に関して初めて述べられた．

●潜在性結核（LTBI），肺結核/肺外結核の診断に関して初めてGRADEシステムによって23の推奨項目が挙げられた．

●これらの推奨事項は明確に強く（strong）推奨するもの（6項目）と条件付き（conditional）で推奨するもの（17項目）を分けており，推奨項目の重要性がよりわかりやすい形となっている．

●本ガイドラインは，診断の基準の位置付けを提供するものではなく，患者個々の情報から結核の診断に至るまで，理論に基づいた決定プロセスを提供するだけのもの，つまり絶対的に従うガイドラインではないという立場を明確に示している．
