Point

●急性冠症候群（ACS）は，ST上昇型心筋梗塞（STEMI）と非ST上昇型急性冠症候群（NSTE-ACS）に分類される．NSTE-ACSは，従来の非ST上昇型心筋梗塞（NSTEMI）と不安定狭心症（UAP）を包括した概念である．

●STEMIでは，発症から12時間以内かつ最初に医療従事者が接触してから90分以内のprimary 経皮的冠状動脈インターベンション（PCI）は有用であり，早期の血行再建を目標として病歴聴取，身体所見の確認，12誘導心電図といった初期対応を的確に迅速に行うことが重要である．

●NSTE-ACSでは，高感度心筋トロポニンを利用して，NSTE-ACSを見落とさないように診断する．症例のリスク層別化を行い，そのリスクに応じた治療を選択する．

●最近は，primary PCIでは橈骨動脈穿刺が推奨されており，薬剤溶出性ステントの使用が，従来型の金属ステントの使用と比較して長期成績が優れている．
