DIC診断に関連するその他の検査と意義

Point

●線溶亢進型播種性血管内凝固（DIC）ではプラスミン・α₂プラスミンインヒビター複合体（PIC）が著増しており，さらに消費性にα₂プラスミンインヒビター（α₂PI）の低下が顕著な場合は，重篤な出血を合併する．

●プロテインC（PC）の低下は血栓傾向を助長し，重篤な臓器障害を合併して予後不良の指標となる．

●敗血症DICでは，プラスミノゲンアクチベーターインヒビター1（PAI-1）が増加し線溶が抑制されるためプラスミン生成が低下し，フィブリン分解産物（FDP）生成量が抑制される．その結果，血栓が溶解されにくくなり臓器障害を合併する．特に，PAI-1著増例では多臓器不全を引き起こし，予後不良となる．

●HMGB-1高値例，感染症型DICにおける顆粒球エラスターゼによるフィブリン分解産物（e-XDP）著増例あるいは低値例は，いずれも予後不良である．
