エコーで腎臓を測る

Point

●超音波検査は腎機能障害のスクリーニング検査として有用であり，正常腎での超音波像は，長径10〜12cm，輪郭は整で，腎皮質のエコー輝度は肝実質と同じかやや低くなる．

●パルスドプラ法で腎区域動脈から得られるRI（renal resistive index）は腎機能と関連し，RI＞0.8では腎機能障害を疑う．

●急性腎障害（AKI）が疑われた場合には，超音波検査で慢性腎臓病（CKD）の除外や腎後性AKIの診断が可能となる．

●CKDでは，腎萎縮，輪郭の不整，皮質のエコー輝度の上昇が確認できる．腎後性AKIでは腎盂は拡張し，腎性・腎前性AKIでは腎臓は腫大する．
