Point

●ループスアンチコアグラント（LA）は血栓リスクであるが，プロトロンビン活性が低下し，時に出血症状を認めるLA症例をLAHPS（LA-hypoprothrombinemia syndrome）という．

●LAHPSでは抗プロトロンビン抗体（anti-FⅡ Ab）が検出されることが多く，クリアランス抗体として出血機序に関与する．

●プロトロンビン活性が正常でも，さまざまな凝固因子活性の低下を認め，時に出血症状をきたすLA症例（LLS）もある．

●出血症状を有するLAHPSでは第Ⅷ因子活性低下や第Ⅷ因子インヒビター活性を認めるため，後天性血友病A（AHA）との鑑別が難しい．
