CRPなどの急性期蛋白の産生機序

●急性期蛋白の発現には,インターロイキン(IL)-6,IL-1,腫瘍壊死因子α(TNF-α)などの炎症性サイトカインが重要である.その際,IL-6は組み合わせ刺激の中心的な役割を果たす.
●IL-6とIL-1によるC反応性蛋白質(CRP)の転写発現は,刺激早期では阻害的に働き,刺激後期では相乗的に働く.
●IL-6とIL-1による刺激後期のCRPの相乗発現は,IL-6刺激の下流の転写因子であるシグナル伝達兼転写活性化因子3(STAT3)とIL-1下流の転写因子であるc-fosの転写複合体形成が必要である.
●CRPは,免疫系の細胞,サイトカイン,転写因子が総動員され誘導される.検査値をみながら病態を理解し,その原因を深く考察することが,よりよい治療につながる.
