直接経口抗凝固薬（DOAC）使用時の凝固関連検査の乖離（AT活性およびPS活性検査試薬）

はじめに

　直接経口抗凝固薬（direct oral anticoagulant：DOAC）は2011年に初めてわが国に導入され，現在までに4種類が使用可能となっている．DOACは標的因子の違いから直接トロンビン阻害薬（direct thrombin inhibitor：DTI．ダビガトラン）と直接活性型第Ⅹ因子（factor Ⅹa：FⅩa）阻害薬（リバーロキサバン，アピキサバン，エドキサバン）に分類される．DOACの作用機序からもわかるように，本剤は一般的に使用されているグローバルな凝固時間〔プロトロンビン時間（prothrombin time：PT）および活性化部分トロンボプラスチン時間（activated partial thromboplastin time：APTT）〕や^1），アンチトロンビン（antithrombin：AT）・プロテインC（protein C：PC）・プロテインS（protein S：PS）などの凝固阻止因子活性やループスアンチコアグラントなどの凝固検査に影響を及ぼす^2，3）．特に血栓症の原因精査の際，すでにDOACを内服している場合は，DOACの種類や測定法によっては凝固阻止因子活性値が偽高値となり，欠乏症の診断を見落とす可能性があるため，十分にその影響について熟知しておく必要がある．

　本稿では，特にAT活性とPS活性に及ぼすDOACの影響について自験例を含めて紹介する．
