細菌性髄膜炎・脳膿瘍

Point

●急性細菌性髄膜炎は内科的緊急疾患であり，速やかに腰椎穿刺を実施して髄液検査結果を基に抗菌薬投与を開始することが必要不可欠である．

●腰椎穿刺前の頭部画像検査は，意識障害や麻痺など占居性病変を疑う場合など限られた状況で実施する．

●髄液検体の一般検査にGram染色やPCR検査などの迅速検査を組み合わせることで，より正確な髄膜炎の病原体診断を“固有名詞”で行うことができる．

●抗菌薬にデキサメタゾンを併用投与することで細菌性髄膜炎の予後を改善させることができる．

●脳膿瘍は可能な限り膿瘍穿刺を行って起因微生物を同定しつつ治療的ドレナージを行うことで，予後を改善できる．
