人工関節周囲感染

Point

●人工関節置換術は患者の生活の質（QOL）向上が期待できる一方で，人工関節周囲感染（PJI）といった術後合併症の危険も潜んでいる．

●PJIは発症の時期によって“早期型”，“遅延型”，“晩期型”に分けられ，それぞれの症状や原因微生物が異なる．

●Parviziらが提唱しているPJIの診断基準は，高い感度と特異度を示し，人工関節感染の診断に有用である．

●治療の基本は抗菌薬治療と外科的ドレナージであり，外科的ドレナージはいくつかの戦術に分かれ，原因微生物によってアプローチや治療期間が異なる．
