はじめに

　従来，急激な腎機能低下を伴う病態は急性腎不全（acute renal failure：ARF）として認識されていたが，2000年代になり，早期診断と早期介入による予後改善を目標とすべく，急性腎障害（acute kidney injury：AKI）という新たな概念が提唱された．この背景には高齢化や慢性腎臓病（chronic kidney disease：CKD），糖尿病などの増加により，侵襲的で高度な治療が適応されるようになりAKIの頻度が急増してきていること，ならびにAKIを起こした場合の長期予後が著しく悪化することが広く認識されるようになったことなどがある．

　本稿では，AKIの定義と病態について診断基準策定に至るまでの経緯や現況を解説するとともに，院内発症AKIと外来（初診）でみるAKIの違い，主に院内発症に多い多臓器不全を伴うAKIとAKI単独の予後の違いについて述べる．
