メランコリー親和型うつ病の治療に伴う脳血流の経時的変化―自動解析による精神障害の画像化

　重症のメランコリー親和型うつ病の，治療に伴う経時的変化を脳血流SPECTでとらえた。極期では頭頂・前頭を中心にした広範な脳血流の低下および脳底を中心にした脳血流の増加という二層パターンが認められた。回復期の初期1か月でまず腹側前頭前野の血流低下が生じ，また左前大脳動脈領域や頭頂を主とした血流が増加した。2か月後，血流低下部位は減少・限局する一方，帯状回，背内側前頭前野および頭頂葉を中心とした脳血流のさらなる増加・拡大の傾向がみられた。9か月後，血流低下部位が増加に転じ，ほぼ全域で脳血流はなおも増加し続けた。うつ病回復に伴う脳血流変化は，頭頂から脳底に向かう方向性を持って緩徐に増加することが示された。
