統合失調症患者における音韻プライミング―陰性症状と音韻的連想の脱抑制との関連

抄録

　Spitzerの主張する統合失調症における音韻プライミングの抑制効果の減衰と促進効果への逆転を追試することを目的として，統合失調症の患者と正常対照群との比較を，音韻プライミングと意味プライミングの双方において試行した。結果として，意味プライミングは患者群・対照群の双方において有意に成立したのに対して，音韻プライミングにおいては，対照群では抑制効果が有意に認められたが，患者群ではこの抑制効果は消失していた。対照群と患者群のプライミング効果の直接の比較においても，意味プライミングにおいては有意差が認められなかったのに対して，音韻プライミングでは有意差が認められた。さらに，プライミング効果に影響を与えることが想定されている臨床尺度に関して重回帰分析を行ったところ，意味プライミングに関しては有意に影響を与えている臨床尺度は見出されなかったが，音韻プライミングに関しては，陰性症状得点が有意に影響を与えており，陰性症状が重篤であるほど，音韻プライミングによる抑制効果は減少し促進効果へと逆転する傾向が認められた。今回のプライミング実験は統合失調症における音韻的連想の脱抑制に関する従来の観察を補足するものであり，統合失調症における言語・思考障害が，従来強調されてきた陰性言語障害(言語性の発動性低下と言語の貧困化＝alogia)だけではなく，観察の切り口を変えればむしろ陽性症状(＝paralogia)として把握し得るような病態を含んでいる可能性を示唆しているものと考えた。
