思春期における自殺企図の臨床的検討―入院を必要とした症例を中心に

抄録

　厚生労働省の最新統計によると，15～19歳の死亡原因のなかで自殺の死亡順位は2位であるが，本邦の思春期における自殺企図の臨床研究はほとんどみられない。そこで，2004年10月1日～2005年11月30日の14か月間に自殺企図で当院救命救急センターを受診した20歳未満の症例のなかで，身体的に入院治療を必要とした34名(連続サンプル)を対象とし，DSM-Ⅳに基づいた精神医学的診断，本人の自殺企図歴，生物学的第一親等者の自殺企図歴と精神障害歴，心理・社会的要因について検討した。性別では88.2％が女性であった。また精神障害が79.4％に認められ，そのうち気分障害の割合が26.5％でもっとも高く，破壊的行動障害および物質依存・乱用による自殺企図の割合はそれぞれ5.9％，2.9％であった。さらに広汎性発達障害を11.8％に認めた。そして自殺企図歴が55.9％にみられ，生物学的第一親等者で精神障害歴がある者は26.5％であった。また心理・社会的要因では家族の問題を有した者の割合が50.0％で最も高かった。欧米での先行研究と比較したところ，性差では女性がより多い傾向にあり，また精神障害では破壊的行動障害と物質依存・乱用が少ない傾向であった。さらに，心理・社会的要因では反社会的問題が少ない傾向を認めた。思春期の自殺企図の再企図防止のためには，本邦における思春期の自殺企図の特徴を把握したうえで治療的介入方法を考察することが重要と考える。
