自傷患者の治療経過中における「故意に自分の健康を害する行為」―1年間の追跡調査によるリスク要因の分析

抄録

　女性自傷患者53例の1年間追跡調査から，治療経過中における自傷行為と過量服薬の出現と患者の心的外傷体験，自傷様態，過去の過量服薬歴，アルコール乱用，過食傾向，衝動的傾向，解離傾向との関連を検討した。その結果，自傷行為がみられた者は58.5％，過量服薬がみられた者は30.2％であり，また自傷に対する縫合処置を受けた者は11.3％，過量服薬に対する医学的治療を受けた者は18.9％であった。さらに自傷に対する縫合処置を受けた者では生活機能の低さと著明な過食傾向が，過量服薬がみられた者では過去の性的虐待歴が特徴的に認められた。これらは治療経過中における自傷行為や過量服薬のリスク要因としての意義があると考えられた。
