リスペリドンによる増強療法で攻撃的な強迫観念が改善した双極Ⅱ型障害の1例

はじめに

　強迫性障害の患者には，十分量のセロトニン再取り込み阻害剤(selective serotonin reuptake inhibitor；SSRI)にても改善しない患者群が存在する。双極性障害に強迫性障害が合併することはしばしば報告されているが¹⁾，炭酸リチウムの投与が，躁状態のみならず強迫症状にも有効か否かについてはまだ十分なデータはない。我々は，fluvoxamineには治療抵抗性であった攻撃的な内容の強迫観念のある強迫性障害の患者が，大うつ病のエピソードの経過中に軽躁病エピソードを併発した症例を経験した。本症例は，lithiumの投与に少量のrisperidoneを追加投与したところ，軽躁病エピソードのみならず，攻撃的な内容の強迫観念も改善したので報告する。なお，本報告にあたっては，患者から文書による同意を得ている。
