はじめに

　このテーマに関してオピニオンを述べられる専門家の多くは臨床精神科医で，臨床の立場からの意見が多いと思われる。私は，国際的な保健医療行政の立場からICDをはじめとする操作的診断についての意見を述べたい。

　WHOにとって診断基準作成の目的は，途上国を含めて世界中から，死亡，発病，有病に関しての信頼できるデータを集め，国際的な疾病動向をとらえ，有効な予防方法，治療方法を策定することである。ICDは，そうした目標を達成するための必要不可欠の道具である。ICD-10の第5章「精神および行動の障害」に掲載してある100の診断名(F00-F99)は，感染症や生活習慣病を含む3,000以上の診断リストの一部である。
