気分安定薬とrisperidoneの併用療法が著効したrapid cyclerの1例

はじめに

　DSM-Ⅳによるとrapid cycler(急速交代型，以下RC)の診断基準は，過去12か月間に少なくとも4回の大うつ病，躁病，混合性，または軽躁性エピソードの基準を満たす双極Ⅰ型またはⅡ型気分障害と規定され，気分障害外来でみられる双極性障害のおよそ5～10％に生じると言われている。

　RCに対する薬物療法として，lithium(Li)，valproic acid(VPA)，carbamazepine(CBZ)，clonazepam(CNZP)，各種ムードスタビライザーの併用療法，甲状腺ホルモンなどが有効¹⁶⁾とされているが，実際の診療では躁病相急性期を気分安定薬のみで乗り切れることはむしろ少なく，抗精神病薬の併用が必要となることも多い。抗精神病薬の中でも，非定型抗精神病薬は統合失調症ばかりではなく，近年，躁うつ病治療においても注目を集め，その有効性を示す二重盲験比較試験結果が報告され^{1,5,12,13,17,18)}，その一部は海外で保険適応を受けている。RCでの非定型抗精神病薬使用については二重盲験比較試験の報告はほとんどないが⁸⁾，オープン試験や症例報告がいくつか海外で報告され^11,14,15)，本邦でもRCにquetiapine(QTP)が著効した症例が1例報告されている⁶⁾。

　今回我々はRCの診断基準を満たす双極Ⅰ型障害の躁病に対して気分安定薬(VPA＋CBZ)とrisperidone(RIS)の併用療法が著効した1症例を経験したため報告する。
