クエン酸フェンタニールにより生じた嘔気に対してオランザピンが奏効した1症例

はじめに

　我々はがんによる疼痛に対して投与された麻薬性鎮痛薬による激しい嘔気のため，疼痛コントロールが困難な症例を経験した。疼痛に対して数種類の麻薬性鎮痛薬を使用したが軽快せず，クエン酸フェンタニールの使用により疼痛は改善した。しかし同時に激しい嘔気が出現して投与量を増加することが不可能であった。この嘔気に対して数種類の制吐剤を使用したが軽快せず，少量のオランザピンを投与したところ，嘔気の改善を認めたので報告する。なお，報告にあたって口頭にて本人の同意を得た。また，科学的考察のために支障のない範囲でプライバシー保護のために症例の内容を変更した。
