摂食障害における社会不安障害

　摂食障害における社会不安障害(以下，SADと略す)の併存の有無によりその臨床像を比較し，摂食障害におけるSAD概念の有用性を検討した。対象は摂食障害患者196例で，半構造化面接によりI軸障害の生涯診断，パーソナリティ障害の診断を行った。その結果，33例(16.8％)にSADの生涯診断を認め，その全例においてSAD発症が摂食障害の発症に先行し，面接時もSADと診断され，32/33(97％)までが全般性SADであった。SAD併存群と非併存群では年齢，現在の摂食障害診断には差がなく，SAD非併存群のほうが最低Body Mass Indexが低く，最大過食回数，嘔吐回数が多かった。一方，SAD併存群のほうが大うつ病性障害，気分変調性障害の併存が有意に多く，自殺未遂，自傷が多い傾向を認めた。SAD併存群のうち，14例に薬物療法と認知モデルの説明を行ったところ，5例(36％)に有効であった。今後，前方視的な検討が必要と考えられた。
