抑肝散の投与で幻視が消失したレビー小体病の3症例

はじめに

　最近，認知症の幻覚や興奮・攻撃性，あるいは焦燥感・被刺激性や異常行動などのBPSD(behavioral and psychological symptoms of dementia)に対する抑肝散の効果が注目されている⁴⁾。また，DLB(dementia with Lewy bodies)やPD(Parkinson's disease)の薬物治療中にもしばしば幻視などの幻覚やせん妄が出現し³⁾，患者のQOLが著しく低下する場合が多い。これらの幻覚やせん妄に対しては従来から抗精神病薬が投与される場合が多いが，しばしば抗精神病薬誘発性のパーキンソン症状が出現し，転倒事故や元来の身体症状の悪化などが問題になる。今回我々は抗精神病薬の投与で幻視を中心とした幻覚は軽快するものの，薬剤性パーキンソニズムが重畳するために，身体症状のコントロールに難渋したDLBとPDおよびPDD(Parkinson's disease with Dementia)の3症例を治療する機会を得た。この3症例とも抑肝散の投与により，薬剤性パーキンソニズムが重畳することなく幻視などの幻覚がほぼ消失した。そこでこの3症例の治療経過を報告し，抑肝散の臨床的意義について若干の考察を加える。なお，抑肝散の投与については，期待される効果や有害事象について，本人および家族に説明し同意を得て実施した。
