大うつ病性障害の入院治療転帰と甲状腺機能との関連

 甲状腺機能とうつ病の間になんらかの関連があることは以前より指摘されているが,甲状腺機能がうつ病の治療転帰にいかなる影響を及ぼすかについては,一定の見解に至っていない。今回我々は,大うつ病性障害の入院患者191名を対象に,人口統計学的背景,症状,入院時の甲状腺機能,薬物療法に関し,診療録に基づいた後方視的調査を行った。その結果,年齢が甲状腺刺激ホルモン(TSH)とは有意に正の,遊離トリヨードサイロニンfT3とは有意に負の相関を示した。年齢で補正した解析より,入院時TSHは退院時Hamiltonうつ病評価尺度総得点と有意に正の相関を示し,また,治療への非反応群は反応群より入院時TSHが有意に高かった。

 以上より,入院時の甲状腺機能が治療転帰の予測因子となり得る可能性が示唆された。
