Paroxetine中止後発現症状―無作為割付による中止方法の3群比較

抄録

　Paroxetineはその有効性と忍容性の高さから，最もポピュラーな抗うつ薬の1つとなっている。しかし，その使用量に比例するように中止後発現症状（discontinuation syndrome）が問題となることがある。今回我々は「paroxetine減量中止戦略」として，A群（20mg/日を10mg/日に減量し，10mg/日を1週間服用後に中止），B群（20mg/日を10mg/日に減量し，10mg/日を1週間服用後さらに5mg/日に減量して1週間服用後に中止），C群（20mg/日を10mg/日に減量し，10mg/日を1週間服用後さらに5mg/日に減量して4週間服用後に中止）と無作為に3群に分け比較検討した結果，中止後発現症状はA群で63.6％，B群で62.5％の患者に発現したのに対し，C群では14.3％の患者にしか発現しなかった。このことから，paroxetineの中止後発現症状を減らすためには，最終的に中止する際に一定の時間をかけてから中止することが有用であることが示唆された。
