晩発性双極性障害の薬物療法に関する後方視的研究

抄録

　中年期以降に発症した双極性障害では，若年発症者との生物学的差異が指摘されており，より適切な治療戦略の確立が求められる。今回我々は，50歳以降に発症した52名の「晩発性」双極性障害患者の薬物治療について，診療録から後方視的に調査した。GAF得点を基準にした評価法では，リチウムは38.7％，バルプロ酸は16.7％，カルバマゼピンは11.1％の有用率を示した。うつ病エピソードに対する抗うつ薬の効果は総じて乏しく，躁転率が有用率を上回っていた。本研究の結果から，晩発性双極性障害の薬物治療にはリチウムが比較的有用であり，抗うつ薬は効果が期待できず，むしろ躁転に注意する必要があると考えられた。
