摂食障害に対する早期介入の現状と今後の課題

はじめに

　神経性食欲不振症(拒食症)，神経性大食症(過食症)などの摂食障害は，若い女性に有病率の多い疾患である。ダイエットが流行している現在，若年女性においては，体重を気にする者は非常に多く，あまり健康に影響のない範囲のダイエットから，診断基準を満たす摂食障害まで，ある程度の連続性がある。このような特徴のため，摂食障害については，「早期」介入という場合に，ダイエットをさせないことを目指すのか，ダイエットはしても体重低下の程度に注意することを目指すのか，診断基準を満たさないことを目指すのか，いくつかの段階がある。これらの段階の違いを意識することは臨床的にはかなり重要なのだが，往々にして，一括りに論じられやすい。また，摂食障害という領域は，精神医学だけではなく，社会学や女性学などさまざまな専門分野で取り上げられ，それぞれの専門による見方の違いがあるが，このことと，摂食障害関連症状の幅の広さがあいまって混乱が生じやすい。ここでは，どのような「介入」，どのような「予防」ならば可能なのか，現状と今後の課題について検討したい。
