はじめに

 倦怠感はがん患者の経験する最も一般的で苦痛な症状の1つであり,がんに関連する倦怠感(cancer-related fatigue;CRF)は疲労感,虚弱,エネルギーの欠乏感で特徴づけられ,休息や睡眠により回復する一般的な疲れとは区別される。CRFはさまざまながんで認められ,日常生活への障害も大きい。抗がん治療中だけでなく治療後にも認められる。がん自体からの影響もあれば抗がん治療の影響としても生じ,時として倦怠感のために抗がん治療を中止せざるを得ない場合もある。近年,CRFの重要性が認識され始め,国際疾病分類第10版修正版-臨床的修正(ICD-10-CM,2007年7月発表)においても新生物(悪性)に関連する倦怠感[R53.0, Neoplastic(malignant)related fatigue]として分類されている。

 本稿では,①がん患者に認められる倦怠感と関連する他の症状について概観し(symptom cluster),その後,②定義・頻度・程度,③評価法,④病態機序,⑤治療法について述べる。病態機序については特に免疫系との関連について注目し記述する。
