Quetiapineによるaugmentationが奏効した身体症状を伴ううつ病の2症例

抄録

　うつ病では抑うつ気分などの精神症状に加えてしばしば身体症状を認めるが，こうした身体症状や心気症状を伴ううつ病は難治化しやすいことが報告されている。我々は身体症状を伴い，抗うつ薬による薬物治療に抵抗を示したうつ病に対し，quetiapineによるaugmentationが奏効した2症例を経験した。2例とも不安・焦燥が強く，身体症状に対しては心気的であったが，quetiapineを75～150mgの少量を追加投与することにより，数日で不安・焦燥が軽快し，身体症状もほぼ消退した。近年quetiapineのうつ病に対する効果が実証されてきているが，今回の経験から，身体症状を伴う治療抵抗性のうつ病に対しても，quetiapineによるaugmentationが有効であるものと考えられた。
