統合失調症と社会脳

近年，発達障害や統合失調症の社会生活を困難にする病態基盤として，社会脳とよばれる脳神経ネットワークが注目されている。「社会脳仮説」では，人を含む霊長類の脳内に，他者の意図や感情を読んだり，人間関係を適切に把握したり，社会の中で適応していくための社会認知を支える神経基盤やネットワークが進化の過程ででき上がったと考える。社会認知の構成要素としては，他者の意図や感情を読む能力（“心の理論”），およびその基をなす，顔の認識，表情認知，視線処理，原因帰属様式（attributional style）などが挙げられ，記憶，注意，遂行機能などの基本的な神経認知とは関連しながらも独立した機能であることが明らかにされている21)。
統合失調症の社会機能的転帰が基本的な神経認知機能と関連が強いことは，よく知られている事実であるが，神経認知機能は社会機能的転帰の20～40％を説明し得るに過ぎないことが指摘されている5)。社会認知の定義からも推測できるように，残りのかなりの部分が社会認知によって規定されている可能性がある。実際，社会認知の重要性は，それが社会機能的転帰とかかわっており5)（図1），神経認知機能と異なる脳領域に社会認知を支える神経基盤があること18)や統合失調症の社会認知機能障害をターゲットとする心理社会的治療が注目されているため，それらについて紹介する。
