強迫性障害にaripiprazoleが奏効した1症例

はじめに

　強迫性障害（Obsessive-Compulsive Disorder，以下OCD）は，反復する強迫観念または強迫行為に特徴づけられ，日常の生活習慣や職業上の機能，社会的活動，他者との関係などを障害し，苦痛を伴う病態である⁸⁾。治療は選択的セロトニン再取り込み阻害薬（selective serotonin reuptake inhibitor，以下SSRI）をはじめとする薬物治療と認知行動療法が主であり，長期的予後は寛解または部分寛解が約半数とされる⁴⁾。またaripiprazole（以下APZ）は，ドパミンD₂受容体の部分作動薬（partial agonist）であることを特徴とする非定型抗精神病薬であり，本邦では統合失調症に対して適応がある。

　今回我々は各種抗うつ薬，非定型抗精神病薬を使用したにもかかわらず症状の改善を認めない難治性のOCDに対して，APZが奏効した症例を経験したので報告する。なお報告にあたって口頭にて本人の同意を得た。さらに科学的考察のために支障のない範囲でプライバシー保護の観点から症例の内容を変更した。
