はじめに

　睡眠時随伴症の中で，高齢者に好発するレム睡眠行動障害(REM sleep behavior disorder；RBD)は，夢体験中に生じる暴力的な行動のために，ベッドパートナーもしくは患者自身が受傷しやすいという横断面での症状レベルの問題とともに，α-synucleiopathyに属する神経変性疾患に発展する可能性を含んでおり，この予防を視野に入れた診断・治療の重要性が高まってきている。本病態の解明は，レム睡眠機構の理解にもつながるものであり，睡眠学にとっての大きな課題の1つであるといえよう。
