認知機能リハビリテーション―統合失調症の治療にどう活用できるか

統合失調症を対象とした認知機能リハビリテーションの効果―これまでの先行研究の検討

1．認知機能リハビリテーションとは

　認知機能リハビリテーションは，cognitive remediation，cognitive rehabilitation，cognitive training，cognitive remediation therapyなどと呼ばれ，認知機能の直接的な改善，もしくは低下している機能を代償する方略の獲得を目指すものであり，生活環境の調整と対比される。後者は身体障害のある人に対するバリアフリー住宅を想像いただければわかりやすいが，たとえば記憶障害のある人に対し，手がかりを与える装置やサポートする人などを環境に配置する工夫である。認知機能の改善にあたっては，認知行動療法の技法や誤りなし学習（errorless learning）などによって，遂行機能などの個々の認知機能の改善を目指す。欧州では，思考スキル（thinking skills）の治療など神経心理学的な概念でとらえられる一方，米国では脳の神経ネットワークの改善といった，より直接的な脳機能へのアプローチとしてとらえられる傾向がある。本論では，認知機能リハビリテーションが統合失調症の治療にどのような有用性を持ち得るか，これまでの文献検討や実践例を通して考察したい。

　ここで述べている「認知機能」とは，神経心理テストで測定され，主に事物処理を行う認知機能である。それに対して社会的機能への影響の点で，社会的認知（social cognition）の重要性を指摘する意見がみられるようになっている。Pennら¹⁷⁾はそのレビューで，事物処理の認知機能障害では，統合失調症の社会機能の障害を十分には説明できないと述べている。Sergiら²⁰⁾は100名の統合失調症もしくは統合失調感情障害の人を調査し，社会的認知と事物処理の認知機能とを分離した2因子モデルがより適合性が高いとしている。Ikebuchiら⁸⁾は63名の統合失調症の人を調査し，社会的問題解決に至る過程において，社会的認知と認知機能とがそれぞれ影響を与えていることを報告している。これらから，社会的認知と，事物処理の認知機能とはそれぞれ連関を持ちながらも，異なる経路において社会的行動に影響を与えていることが考えられる。社会的認知や，自己に対する認識の障害は臨床上重要であるが，本論では議論の焦点を限定するために，認知機能と書くときには，神経心理テストで測定される事物処理の認知機能を指すこととする。ただし，社会的認知も含めた広義の認知機能についても，統合失調症の治療を考えるうえで重要性が高いことは，強調しておきたい。
