感情障害を対象とした集中的通院による認知行動療法プログラム―米国マックリーン病院における実践

はじめに
筆者は2008年よりマサチューセッツ州マックリーン病院にてBehavioral Health Partial Hospital Program(以下，BHP)を学ぶ機会を得た。当病院は1811年より続く全米最古の精神科専門病院である。またHarvard University，Massachusetts General Hospitalの提携病院かつ研究機関であり，多くの優れた治療プログラムと研究成果を提供し続けている19)。BHPは，認知行動療法を全面的に用い，うつ病，双極性障害，不安障害，人格障害といった幅広い疾患を対象とする当病院独自の短期集中的通院プログラムである。本邦でも，感情障害圏の治療困難例，気分障害を伴う人格障害患者など，薬物療法に限界のある患者への精神療法的手法の需要は大きい28)。BHPはこれら疾患群に対しても，優れた治療効果を示している5,24)ことから，当プログラムを紹介するのは意義深いことと思われる。本論文では，BHPの具体的な内容とその特徴を詳しく紹介し，本邦で同様のプログラムを施行する際の課題についても考察する。
