特集 児童期における精神疾患の非定型性―成人期の精神疾患と対比して

はじめに
強迫性障害(obsessive-compulsive disorder；OCD)は，強迫観念と(もしくは)強迫行為によって規定されており，それらによって正常思考や日常生活動作が脅かされることをもって，強迫性障害とされる。ただし注意する点としては，子どもにおいては，発達過程にみられる正常の範疇に属する強迫から強迫性障害とされる強迫までスペクトラムとして理解することが可能であり，児童期にみられる正常レベルの強迫は子どもが発達するために経過しなければならない課題ともいえる側面を持っている点である。すなわち障害とはいえない強迫症状があり得るということになるので，「それが悩ましく，長時間続き，社会活動を害していること」をもって障害と特定されている。本稿においては児童期発症OCDと成人期発症OCDとの比較を含め，子どものOCDについて述べていきたいと考える。
