摂食障害患者の変化への段階と早期治療脱落

抄録

　摂食障害患者の治療意欲と早期治療脱落の関係について検討した。対象は，初診時に摂食障害と診断され同意を得られた女性患者152例で，神経性食思不振症(AN)61例，神経性過食症(BN)81例，特定不能の摂食障害(EDNOS)10例にthe stage of change questionnaire日本語版を用いて変化への段階を評価した。64例(42.1％)が通院5回以下で治療を中断し，BN群の脱落率が53.1％と高かった。変化への段階のうち，熟考段階では自己評価の低さが，行動段階ではやせ願望，過食衝動，内界への気づきが脱落と関連している可能性が示され，個々の患者の治療意欲の程度に合わせた対応の必要性が示された。
