認知矯正療法による予後改善―NEARを中心に

はじめに
認知矯正療法は認知機能障害を改善することで統合失調症の予後の改善を目指す心理社会的手法である。認知機能障害は前駆期から認められ，急性期に悪化するが安定期には顕著な改善は認められない。統合失調症のおよそ8割が認知機能障害を示し，その深刻さは健常者の平均と比較して1.5標準偏差ほど低いといわれるように顕著である。認知機能障害は社会機能，自立生活機能，就労機能と多方面にわたり影響を及ぼすことから，予後改善につながる重要な治療標的である。認知矯正療法はこれら機能の基盤になる注意，記憶，問題解決，処理速度などの認知機能を訓練する。認知矯正療法には複数の種類があるが，本稿ではNEAR(Neuropsychological and Educational Approach to Cognitive Rehabilitation)を中心に解説する。
