はじめに
世界保健機関(World Health Organization, WHO)は，医療資源の少ない国でも正確な診断を可能にするような国際疾病分類(International Classification of Diseases, ICD)の改訂作業を進めており，新しいICD-11「精神および行動の障害」診断ガイドラインを2014年以降に導入する予定である。現在のICD-10 43)は細分類を追求するあまり，利用者側の観点が犠牲にされたとの反省があり，医療従事者だけでなく，患者やその家族にとっても使い勝手のよいものに簡略化する予定であるという。わが国では行政や保険業務，疾病統計などでICDを使用することになっており，ICD-11への改訂の影響はきわめて大きい。
一方，アメリカ精神医学会(American Psychiatric Association, APA)で作成している「精神疾患の分類と診断の手引き」(Diagnostic and Statistical Manual, DSM)は，主要な雑誌に掲載される精神医学の研究論文ではICDよりもはるかに多く使用されている。現在のDSM-Ⅳ3)とICD-10の不一致は精神医学研究の混乱の原因となっており，1999年より開始されたDSM改訂作業はWHOも参加して進められている。2010年2月には改訂草案がウェブ上で公開され，2013年には新しいDSM-5を出版する予定であるという。
国際抗てんかん連盟(International League against Epilepsy, ILAE)では1981年にてんかん発作の国際分類6)，1989年にはてんかん症候群分類7)，そして2007年にはてんかんに合併する神経精神障害の分類試案21)を公表した。その中には，てんかんに特異的な精神・行動障害のカテゴリーも提案されている。しかし，ICDやDSMとの整合性は考慮されておらず，てんかん特異的障害の妥当性をめぐるその後の議論も十分でないように思われる。表1には精神・行動障害とてんかんの国際分類に関して，これまでの経緯をまとめた。本稿ではてんかんにみられる精神・行動障害，とりわけてんかんに特異的な精神・行動障害分類についての現状を報告する。
