うつ症状を呈したBasedow病の1例

はじめに

　急性・亜急性に出現した精神症状に対して，治療方針を立て予後を見定めていくためには，治療初期にその症状が治療可能な身体疾患による外因性の病態に基づいていないか，しっかり鑑別することが基本かつ重要となる。甲状腺機能亢進症の代表的疾患であるBasedow病の有病率は1,000人に1～2人程度であり，身体症状とともに多様な精神症状を呈することが報告されており⁸⁾，急性・亜急性に精神症状が出現した場合，本症も考慮に入れる必要がある。今回，thiamazoleに対する治療反応性から，Basedow病に伴う精神症状としてうつ症状を呈した可能性がある症例を経験したので，若干の考察を加え報告する。

　なお，症例報告に際して本人より同意を得た。また，個人情報保護に配慮して，事実に影響を与えない範囲で適宜修正を行った。
