大学生における双極性気分変調に関する報告―自記式簡易スクリーニングを用いた検討

若年者の双極性気分変調は，多様な心理行動上の問題と関連する。本研究では，健康診断の一環として行っている自記式調査に，双極性気分変動をスクリーニングする2つの質問（高揚気分と易刺激性）を加え，抑うつや精神病的症候，食行動異常などとの関連について解析を行った。対象は春の健診を受検した5,425名の大学生で，うち有効回答者は在校生4,138名，新入生1,263名だった。記入時の2週間前後に双極性障害の症状項目をしばしば満たす割合は，在校生で高揚気分54名（1.3％），易刺激性70名（1.7％），両者10名（0.2％），新入生では高揚気分13名（1.0％），易刺激性32名（2.5％），両者6名（0.5％）だった。重回帰分析の結果，両質問項目得点は，感情易変性やイライラ感，自殺念慮，興味・関心の減退，食欲睡眠の変化などの抑うつ症状と強く関連し，幻聴や関係念慮，食や体型のこだわり，生活障害度とも有意な相関を示した。双極性気分変調の時点兆候は一般大学生の0.3～0.5％に認められ，こうした事例では，混合状態や躁うつ急速交代などの情動易変性，精神病的症候，食行動異常の併存にも注意して対応する必要がある。
