Rivastigmineにより幻視が消失したレビー小体型認知症の1例

抄録

　海外ではレビー小体型認知症(DLB)の認知症周辺症状(BPSD)に対するコリンエステラーゼ阻害剤(CheI)の有用性に関する報告が散見される。今回我々は87歳の女性，主訴が幻視のDLB患者に対してCheIを主剤に治療を行った。しかしdonepezil，galantamineでは副作用により中止せざるを得なかったが，rivastigmineパッチ剤では副作用なく治療することができ，幻視などのBPSDの改善を認めた。経口剤に比べてパッチ剤では安定した血中濃度を維持することで副作用を認めず使用できたと考えられた。本症例を通してrivastigmineはDLBのBPSD治療において第一選択のひとつとなり得ると考えた。
