Behavioral and Psychological Symptoms of Dementiaと思われる諸症状を呈した混合型認知症にramelteonの追加投与が奏効した1例

はじめに

　超高齢化社会のわが国においては，認知症のBehavioral and Psychological Symptoms of Dementia(BPSD)¹⁾は，介護者の負担のみならず，医療費の増大といった医療経済上でも議論となるなど大きな社会問題となっている^2,12)。我々精神科医がBPSDの治療を行う場合，抗精神病薬をはじめとする向精神薬を使用せざるを得ない場合が多いが，しばしば副作用などのために治療に難渋する場面に直面する。さらに2005年のアメリカ食品医薬品局(FDA)が抗精神病薬投与により，高齢患者の死亡率が上昇する可能性を警告してから，その使用においては，より慎重な検討が求められている。

　今回，我々は認知症のBPSDと思われた諸症状に，メラトニンアゴニストであるramelteonの追加投与が奏効した症例を経験したので，文献的考察を交えて報告する。なお，ramelteonの投与については，その期待される効果・予想される副作用などを家族に十分に説明し，同意のもとに投与した。また報告にあたっては家族の了解が得られており，倫理面に配慮して趣旨に影響を与えない範囲で内容を一部変更した。
