トウレット障害を併存する強迫性障害の臨床像：第1報―併存による臨床像への影響に関する多角的検討

トウレット障害（Tourette's disorder；TD）を併存した強迫性障害（obsessive-compulsive disorder；OCD）患者20例の臨床像について，年齢・性別を一致させたチックの生涯罹病を有さないOCD患者23例を対照に比較，検討した。その結果，TD併存群では，OCDの早発や強迫症状に対する洞察不良，対称性・正確性の追求や繰り返しの儀式行為，溜め込みが高率といった症候学的特徴が認められた。またTDを併存すれば社交不安傾向が有意に高度であるなど，対人関係機能に悪影響が及び，チック症状の重症化に関連する可能性が示唆された。これらの結果は，DSM-5に導入されたチック関連性サブタイプの特異性，あるいは臨床的有用性を支持するものと考えた。
