短期間に再発した抗NMDA受容体脳炎の男性例

抄録

　短期間に再発した抗N-methyl D-aspartate受容体抗体に関連した脳炎(以下，抗NMDA受容体脳炎)の男性例を経験した。感冒症状の後に統合失調症様症状，意識障害，強直間代発作が認められ，免疫療法や向精神薬の投与により症状は改善し，3か月後に退院となった。しかし，退院2か月後に感冒症状，抑うつ症状，統合失調症様症状，意識障害を呈し再発した。本疾患の男性例は少なく，本邦での男性例は数例しか報告されていない。また，女性例では卵巣奇形腫の合併が多いことが知られているが，男性例は女性例と比較して腫瘍の合併は少なく，非腫瘍合併例では腫瘍合併例よりも再発が多いと報告されている。本症例の臨床経過を示し，文献的考察を加えて報告する。
