rTMS治療前後にNIRS検査を施行した3症例

抄録

　経頭蓋磁気刺激(transcranial magnetic stimulation；TMS)はうつ病治療として応用されており，その治療法として，左前頭前野への高頻度反復性TMS(repetitive TMS；rTMS)および右前頭前野への低頻度rTMSがある。また近赤外線スペクトロスコピィ(near-infrared spectroscopy；NIRS)検査では，主に大脳皮質のヘモグロビン濃度変化が測定可能である。

　当院にてうつ病治療として，左背外側前頭前野への高頻度rTMSと右背外側前頭前野への低頻度rTMSを行い，その前後でNIRS検査を施行した3症例を経験した。各症例とも治療前後でNIRS検査における前頭部，左右側頭部関心領域の酸素化ヘモグロビン平均波形の課題中の積分値の上昇がみられ，それと同時に症状の改善がみられた。

　3症例を通して，rTMS治療後に症状の改善とともにNIRS検査での酸素化ヘモグロビン平均波形の課題中の積分値の上昇を認めたことから，rTMSの治療効果判定にNIRS検査を使用できる可能性が示唆された。
