異なる機序の抗うつ薬3剤でSIADHを呈した反復性うつ病性障害の1例

抄録

　今回我々は，異なる機序の抗うつ薬3剤で抗利尿ホルモン不適合分泌症候群syndrome of inappropriate antidiuretic hormone(SIADH)が生じた1例を経験したので報告する。症例は78歳，女性。反復性うつ病性障害，現在中等症エピソードの診断でX年6月に入院となった。入院中，Na濃度がmirtazapine 7.5mg/日内服下で128mEq/l，escitalopram 15mg/日内服下で128mEq/l，duloxetine 40mg/日内服下で119mEq/lと低Na血症を来しSIADHと診断された。抗うつ薬投与を中止するとNa 135mEq/l前後で経過した。異なる機序の抗うつ薬3剤でSIADHを来した症例は，海外も含めてこれまでに報告がない。高齢者に抗うつ薬を投与する際には作用機序に関わらず低Na血症に注意しながら投与する必要があると考えられた。
