電気痙攣療法に関連した心静止についての検討

　電気痙攣療法に関連した心静止について，その危険因子や治療的対応に関する考察を加えて報告する。当科での心静止の発生頻度は23例中6例26.1％で，ECT施行回数192回中12回6.25％であり，6例中2例が心静止，およびそれに伴う頻脈性不整脈によりECTの継続を断念した。先行研究と同様に年齢，性別，基礎疾患などは心静止の発症予測因子となり得なかった。しかし，けいれん閾値が上昇して有効なけいれん発作を誘発するためにサイン波治療器が必要となった患者において，有意に(オッズ比14：95％ CI1.47-133)心静止の発生頻度が高かった。Atropine sulfateによる前処置やremifentanilの併用によりけいれん閾値の上昇を抑えることで心静止発生を減少させうる可能性が考えられ，今後さらなる検討が望まれる。
