Aripiprazoleの少量追加により，blonanserinに起因すると考えられた高プロラクチン血症と無月経が改善した高用量抗精神病薬服用中の慢性統合失調症患者の2症例

抄録

　Blonanserin(BNS)とquetiapine(QTP)もしくはolanzapine(OLZ)の2剤を併用しているchlorpromazine換算1,000mg以上の高用量抗精神病薬投与中の慢性統合失調症患者に対し，抗精神病薬の減量，単剤化，高プロラクチン(PRL)血症の改善を目的にaripiprazole(APZ)へのスイッチングを試みたところ，少量からのAPZの上乗せ投与中に高PRL血症と無月経の改善がみられたので報告した。QTP，OLZは高PRL血症を来しにくいが，BNSは20～30％に高PRL血症を起こすことが報告されており，服用薬剤の組み合わせから考えて，BNSにより引き起こされた高PRL血症の可能性が高いと推測された。APZよりドパミンD₂受容体親和性の強いBNSを高用量服用している患者に対し，少量のAPZの上乗せ中に改善が起こったことは興味深い現象であった。高PRL血症は患者から自発的に問題を訴えないが，性機能障害，骨密度の減少，月経異常，乳癌リスクの上昇の可能性が示唆されており，軽視できない有害事象を来しうる。BNS，APZとも非鎮静系で副作用の少ない薬剤であるが，その薬理作用から，高PRL血症に対してはAPZの使用は有用であると考えられた。なおスイッチングは順調であり，精神症状の悪化や新たな有害事象の発現はない。
